New Patients

So you want to schedule a new patient appointment with Dr. Wood? Here is where you begin the process.

There are a couple of ways for you to reach us to get started, and they are listed below.

Be aware that before we schedule any new patient appointment, we do require that our "New Patient Registration Form" and our "Endocrine Information Sheet for New Patients" be fully completed and returned to us.

Ways to reach us to to schedule a New Patient appointment:

  • Email us the "New Patient Registration Form" and "Endocrine Information Sheet for New Patients":               ***FASTEST  WAY TO GET ON THE SCHEDULE***

Step 1:      Download: our New Patient Registration Form and complete it with your information. 

NOTE: Some fields may not apply to you -- please complete all fields that do apply to you.

Step 2:     Download: our Endocrine Information Form for New Patients  and complete it.

This is the most important piece of information necessary to schedule a new patient appointment. Incomplete forms may need to be returned and will delay securing an    appointment date and time. 

Step 3:     Email: the completed forms to Appointments@mipeds.org          

Step 4:     Schedule: Someone from our office will call you to schedule as soon as possible.

However, you are welcome to call us at 734-469-4775 to confirm receipt of the forms and ask to schedule the appointment.

Be sure to include the patient's name, date of birth, your name (if you are the parent or guardian) and relationship to the patient, and a phone number to reach you.
One of our staff will contact you to continue the process.

  • Register on our Patient Portal

Follow our Patient Portal Registration Instructions to get signed up on our Patient Portal

Make sure to complete your profile after logging in -- It will make scheduling your appointment faster.

Once we receive the portal registration, one of our staff wll contact you to continue the process.
